Obtaining a Billing Number
A billing number is required for all providers who will practice in your division. To obtain a billing number packet, please contact Osman Akhtar ( , 726-0973) and he will provide you with a copy of a packet that the new provider must complete. Once the packet is completed please return to Osman and he will coordinate with PSO to enact the billing number.
Preferably Osman should be notified as early as possible as the minimum lead time after receipt of a completed packet is three months for Medicare and MediCal to assign a provider number. In addition, a billing number cannot be assigned until the provider has hospital and clinic privileges (both may not be required).
